Do you think the Document Pyramid remains valid today? If not, what would you change, remove, or add?

Intro

Twenty years ago, with the release of ISO 9000:2000, a document Pyramid was applied. Today we use the term: Documented Information.

L1 - Why? Policies - QMS Manual, Org's commitment
L2 - What? When? Where? Who? Procedures - Overall Process Description
L3 - How? Work Instructions, Forms, Specifications, Drawings - Detailed transformation directions
L4 - Objective Evidence. The Records from L1, L2, L3 of the work completed and results.

The Document Pyramid shows the waterfall effect of information. With increased updates to documents and records in the lower tiers. All tiers produce objective evidence (records) with retention periods.

In the following posts, we’ll dive into each tier of the pyramid to understand what should be included. This structure provides a strong base for your Quality Management System (QMS) and will meet Documented Information requirements of many ISO standards.
